After six months of intervention, the self-management ability score of the observation group was significantly higher than that of the control group. The creatinine, urinary microalbumin creatinine ratio, systolic blood pressure, diastolic blood pressure, and BMI values were significantly lower than those of the control group, indicating that the system can improve the self-management ability of patients with chronic kidney disease and delay disease progression.
